Core Eight-Step Process

Step 1: Contact a Chinese Hospital First

This is the most important step. Without a hospital invitation or acceptance letter, the visa application lacks a foundation. The correct order is hospital first, visa second. Choose a hospital, contact its international department or international patient center, submit your medical records, and use formal translation if the records are not in Chinese. The hospital evaluates the case and issues a Hospital Invitation Letter. The letter should include the hospital seal, physician signature, and ideally bilingual Chinese-English wording. Hospitals with relatively strong experience issuing acceptance letters include PUMCH International Department, Huashan Hospital International Medical Center, the special medical center at the First Affiliated Hospital of Sun Yat-sen University, United Family Healthcare, and Shanghai Jiahui International Hospital.

Step 2: Prepare the Material Checklist

• Original passport: Valid for at least six months, with at least two blank pages
• Visa application form: Complete online at visaforchina.org and print it
• Passport photo: 33 by 48 mm, white background, taken within six months
• Hospital Invitation Letter: The core document that visa officers care about most
• Translated medical records: Use a qualified translation company; do not translate critical records yourself
• Hospital appointment proof: Supports inpatient or surgery arrangements
• Proof of financial capacity: Preferably covering twice the estimated medical cost
• Round-trip flight booking: A reservation is enough; do not necessarily issue tickets early
• Accommodation proof: Hotel booking or host accommodation proof
• Insurance proof: Strongly recommended; should cover medical care inside China

Step 3: Complete the Online Form

Use visaforchina.org and select private affairs under the S visa category. For purpose of visit, answer truthfully as private affairs and explain that you are going to China for medical treatment. Expected stay should match the treatment cycle assessed by the hospital. For the China contact, use the hospital international department contact. For a first application, choose single entry unless there is a reason not to.

Step 4: Submit the Application

Submit through the Chinese Visa Application Service Center in your country. In most countries, applicants do not go directly to the embassy. The process is appointment, in-person attendance, fingerprint collection for ages 14 to 70, payment, and receipt collection. Fees may include USD 30 to 180 for the visa plus USD 15 to 30 for service fees.

Step 5: Wait for Approval

• Regular: 4 to 5 working days
• Express: 2 to 3 working days
• Rush: 1 working day

Step 6: Collect the Visa

Check the visa page carefully for category, validity, duration of stay, and number of entries.

Step 7: Enter China

Carry your passport and visa, the original hospital invitation letter, bilingual medical records, insurance card or policy, and proof of return-ticket reserve.

Step 8: Complete Accommodation Registration Within 24 Hours

This is the easiest step to miss and can have serious consequences for extension and future entry. Failure to register can lead to fines of CNY 2,000 to 5,000, inability to extend the visa, and possible impact on future entry.

• Hotel: The front desk usually completes it automatically; Immediate
• Hospital: Hospital security or administration department; Within 24 hours
• Private residence: Local police station; Within 24 hours; this is especially important

Visa Extension When Treatment Exceeds the Visa Period

Never wait until after the visa expires. Apply about 30 days in advance. Ask the hospital to issue a written medical-condition certificate stating that continued treatment is needed. Then go to the local district or city Public Security Bureau Exit-Entry Administration. The fee is about CNY 160, and processing usually takes 7 to 15 working days.

Suggested Timeline for Heart Surgery

T minus 90 days: contact the Chinese hospital and submit medical records. T minus 75 days: obtain the hospital acceptance letter. T minus 60 days: complete the online form and prepare materials. T minus 45 days: book the visa center appointment and submit. T minus 40 days: obtain approval. T minus 30 days: buy tickets, book accommodation, and confirm insurance. Day 0: enter China and complete accommodation registration within 24 hours. Day 2: begin care. At the end of treatment: extend if needed or leave China.

Common Critical Mistakes

• Believing there is a separate medical visa category: Wastes time searching for something that does not exist
• Underestimating treatment duration: The granted stay may be too short for treatment
• Forgetting 24-hour accommodation registration: May prevent extension and lead to fines
• Overstaying the visa: Can trigger daily fines of CNY 500 and possible blacklist consequences
